This position is a full-time, permanent, salaried position.
Duties and
Responsibilities:
Assist clients with case management and case updates.
Work with the caseworker to ensure that clients are receiving the highest level of care.
Provide case management and case updates to clients.
Maintain a high level of communication with clients.
Participate in client visits with caseworker and other staff.
Assist in the development of service plans for clients.
Participate in the selection, evaluation, and training of new staff.
Participate in the development of service plans for clients.
Assist with the coordination of services for new clients.
Participate in the development of service plans for long-term clients.
Participate in the selection, evaluation, and training of other staff members.
Other duties as assigned.
Required Skills and Abilities:
Ability to work cooperatively with all staff members including co-workers, supervisors, and other clients.
Ability to handle multiple tasks at one time while remaining organized and paying attention to detail.
Ability to read, write, speak, and understand English sufficiently to perform the job duties and satisfactorily answer questions from supervisors or other staff members when necessary.
Ability to write legibly; complete job applications; complete intake forms; prepare client reports; collect client contact information; maintain accurate records; follow up on referrals; document activities; update client records as needed; respond effectively to client inquiries regarding services available; meet required standards for handling confidential information (i.
e.
, HIPAA) regarding client matters (i.
e.
, medical records, mental health services, etc.
) Ability to work cooperatively with all staff members including co-workers, supervisors, and other clients.
